Of course, even such a big monstrosity with layers and layers of accretions can work very well for people, and in cases. But I think the times when it doesn't work, or when a thing it claims to be able to do fails to happen, give us clues as to the places that those patches didn't fix a thing, but just papered over it. So if therapists disagree very deeply about which is more important preventing harm or providing affirmation or they disagree about what harm is, or the source of knowledge from which such decisions are to be made, or even what the reality is that they are dealing with, then that is one of the places that suggests something got papered over but not resolved.
